We are looking for a Licensed Practical Nurse (LPN) to help take the best care of our community and enjoy a rewarding career in an environment that values you and contributes to your continued success.

• Provide compassionate and effective care and treatments to residents.
• Work with a team of professionals to ensure residents' comprehensive needs are met.
• Review routines, calendar of events and resident needs.
• Ensure medications and supplies are ordered, in stock and counted.
• Ensure all charting and record keeping is accurate and complete.
• Attend meetings and trainings as required.
• Promote the Mission, Vision and Values of Transforming Age, and our Culture of Excellence in all areas of responsibility.
• Perform other duties as assigned.

• Graduate of an accredited LPN program.
• Active LPN license in the state, in good standing.
• Experience in a nursing home (assisted living or long‑term care preferred, but not required).
• Flexibility to work occasional weekend/holiday as needed.
• Knowledge of DHS regulations.
• Excellent interpersonal and communication skills and ability to work well and collaboratively with team members and external individuals.
• Completion of background check, drug screen. Other pre‑employment screenings may include physical, TB test, and fingerprinting per state/local guidelines.
