The Family Preservation Case Manager is intended to work with eligible families on a short-term basis. The Family Preservation Program focuses on improving issues identified by the family, which may include safety and well-being factors, reducing risk factors, increasing protective factors, strengthening parenting skills and abilities, and facilitating family stabilization while preventing out-of-home placement of children. The Family Preservation Case Manager works with families to reduce or eliminate barriers to child safety and ensures active efforts are being made while children can remain in their home.

Case management services will be provided for cases determined to be low risk for immediate harm to the child’s safety and well-being. Prevention plans, individualized case plans, resources, and referrals will assist families with their needs and barriers. Services are provided in-home or within the community and efforts are focused on building rapport and trust to engage families for a more meaningful, long-term change.

• Associate degree in human services, or other related field, is required.
• Bachelor’s degree in education, human services, business, and/or organizational development, or a combination of secondary education and training-development experience totaling four or more years, is preferred.
• A minimum of two (2) years of prior experience working with at-risk children and families is required.
• Experience as a case manager providing case management or service delivery to children and families in a public, private, or tribal program is strongly preferred.
• Knowledge and experience working with Native American communities, culture, customs, and cultural law.
• Ability to work independently with little or no supervision.
• Ability to communicate effectively, written and verbal, is required and demonstrated ability to engage people to build rapport, trust, and confidence.
• Strong interpersonal, organizational, managerial, and problem-solving skills as well as excellent time management and observational skills.
• As a key-stakeholder in planning multi-disciplinary interventions must be able to demonstrate the ability to plan, coordinate, implement and evaluate and modify family prevention plan.
• Refer families to Mille Lacs Band community services as needed to support meeting the prevention plan, or plan of care.
• Build and maintain professional working relationships with the HHS team, OSG, state, county, and local agencies that are working in collaboration to manage a multi-disciplinary plan of care.
• In lieu of education, special consideration to include experience. A minimum of 3 years of experience in child welfare or human services-related field, or a combination of both.
• Must have a current valid Minnesota Driver’s license.
• Must be insurable under the Mille Lacs Band Drivers Insurance policy.
• Must pass a pre-employment drug and alcohol test.
• Must pass a background check.
• As a mandated reporter, one must understand the process and assessment parameters of mandated reporting.

• Provides case management services for children and families that address and mitigate family safety and risk issues, with caseloads limited by Departmental policy.
• Coordinate daily with the Family Preservation Support Specialist staff to ensure continuity of care for each family the program is aiding.
• Works with each family to develop a prevention plan for children and families that address and mitigate family safety and risk issues that led to contact with Family Services, exercising the Signs of Safety model.
• Coordinates with families to receive services in their homes and within the community.
• Provides resources and referrals in the community based on client and family needs assessment.
