We are seeking a highly motivated and compassionate LPN MDS Coordinator to join our team at Sunset Ridge Healthcare and Rehabilitation Center in Bloomsburg, Pennsylvania. As an LPN MDS Coordinator, you will be responsible for coordinating the care of our residents by developing, implementing, and evaluating individualized care plans. You will work closely with our interdisciplinary team to ensure that each resident receives the highest level of care and attention. Your exceptional communication and organizational skills will be essential in maintaining accurate and timely documentation, ensuring compliance with regulatory standards, and participating in Quality Improvement initiatives.

• Develop, implement, and evaluate individualized care plans for each resident
• Coordinate with the interdisciplinary team to ensure seamless care delivery
• Maintain accurate and timely documentation of resident assessments, care plans, and progress
• Ensure compliance with regulatory standards and quality of care initiatives
• Participate in Quality Improvement activities to improve resident outcomes
• Collaborate with other departments to resolve issues and improve communication

• Current licensure as a Licensed Practical Nurse (LPN) in the state of Pennsylvania
• 1+ years of experience
• Ability to work in a fast-paced environment with multiple priorities
• Excellent communication, organizational, and problem-solving skills
• Strong attention to detail and ability to maintain confidentiality
