Job Overview
Position:
Physical Medicine and Rehabilitation (PM&R) Nurse Practitioner / Physiatry Clinician
Location:
Bloomsburg, PA. Designated to a single skilled nursing facility or post-acute rehab center. 16-hour, part-time W‑2 role.
- Job Type: Part-time (16 hours) – 8‑hour shifts with flexible hours around 9AM–4PM, weekdays Monday–Friday.
- Pay: Starting at $75.00 per hour.
- Schedule Flexibility: Hours can be adjusted within the 8‑hour shift framework.
- Supplemental Pay: Bonus pay available.
- Commute/Relocate: Must reliably commute or plan to relocate to Bloomsburg, PA before starting.
- Work Location: Bloomsburg, PA.
- Flexibility in hours, typically 9AM–4PM.
- Full‑time virtual scribe assistance for documentation ($1,368/month value).
- 401(k) plan with 2–3% annual employer contribution.
- 100% malpractice insurance coverage.
- Opportunities to earn a minimum of 33 CEUs at no cost through the company.
- Access to CRC's Physiatry certification and CEU credits.
- Company‑provided laptop.
- Provide specialized Physical Medicine and Rehabilitation (PM&R)/Physiatry services as a rehab consultant.
- Represent CRC in your facility through strong clinical leadership and collaboration with the interdisciplinary care team.
- Collaborate with PCP teams to manage common SNF conditions such as CVAs, progressive neurological conditions, and postoperative orthopedic surgeries.
- Work alongside PT, OT, and SLP teams to improve patient function and quality of life.
- Follow therapy patients to track their progress.
- Assess and collaborate on managing barriers to therapy, including joint or neuropathic pain, spasticity, neurogenic bowel, neurogenic bladder, and more.
- Attend multidisciplinary meetings and bridge the gap between therapy and internal medicine.
- Engage in culture‑building opportunities and ongoing education, including monthly didactic lectures, virtual regional team meetings, and annual regional dinners.
- Passion for post‑acute healthcare and serving the geriatric population.
- Experience in SNF or post‑acute settings (preferred).
- Confidence to work autonomously.
- Desire for continuous improvement and openness to change and growth.
- Strong interpersonal skills and ability to understand needs of facility partners and patients.
- Empathy and compassion.
- Commitment to invest in company culture.
- Registered Nurse Practitioner/APRN license.
- Pennsylvania state license.
- Legal ability to work in the United States.
- Positive attitude and desire to deliver best patient care.
- Experience in post‑acute space preferred.
