• Providing direct patient care in collaboration with our interdisciplinary team
• Assessing patient needs, developing, and implementing individualized care plans
• Monitoring patient progress, adjusting care plans as needed
• Communicating effectively with patients, families, and healthcare team members
• Maintaining accurate and detailed records of patient care and treatment
