Personalized Health Partners is seeking a Nurse Practitioner for adult and geriatric primary care across our Southbrook Healthcare Center network in Ohio. This full-time role includes a $15,000 sign-on bonus, a base salary plus productivity bonuses, and a Monday–Friday, 8a–5p schedule with no on-call, weekends, or holidays.
The NP will work in a high-touch, patient-centered model with a collaborative care team and opportunities for telehealth and ongoing education to support seniors in SNF,
