At COA, PRN Licensed Social Workers and Registered Nurses conduct person-centered assessments that help older adults remain independent. You’ll make a meaningful impact while enjoying flexibility, autonomy, and the satisfaction of mission-driven work.

• Conduct telephonic and in-person assessments to determine program eligibility
• Coordinate wraparound services for individuals with complex medical, behavioral, or social needs
• Educate clients and caregivers on available resources and care planning
• Advocate for clients facing health and safety risks or potential institutionalization
• Collaborate with internal teams, healthcare providers, and community partners

• Active Ohio LSW or RN license in good standing
• At least one year of experience in medical social work, home health, or geriatrics
• Strong understanding of aging-related issues and community resources
• Valid driver’s license and reliable transportation
• Excellent communication, empathy, and clinical judgment
• Above-average technology skills and adaptability
