Job Description

Step into a meaningful role within our Hospice Care Team as a Registered Nurse dedicated to supporting patients and families during some of life’s most vulnerable moments. You’ll serve as a trusted resource for nurses, physicians, caregivers, and loved ones, helping maintain comfort, dignity, and continuity of care.

Location

Hospice of Cincinnati – Requires travel. This position involves driving to the patient’s location and doing SIA visits for our Hospice patients. SIA refers to Service Intensity Add‑on visits, which are additional visits provided during the last seven days of a patient’s life. These visits are intended to provide more intensive and focused care during the final stages of life.

Work Schedule
  • Part Time 8am‑8:30pm
  • Day Shift – Weekend Only (Saturday and Sunday)

Job Overview

This position applies the nursing process in providing direct and indirect holistic care to patients and their families. It collaborates, in a collegial manner, with physicians and other health‑care team members to meet the patient’s needs, and may delegate certain aspects of care to those under direct supervision within the scope of nursing practice. The role requires weekend hours and may be on‑call during evenings and nights, so flexibility is essential.

Job Requirements
  • Associate’s Degree or Diploma in Nursing
  • New hires required to obtain BSN within 5 years of hire
  • Basic Life Support for Healthcare Providers (BLS)
  • Registered Nurse license
  • Knowledge, judgment, and skills derived from the Principles of Biological, Physical, Behavioral, Social, and Nursing sciences to meet complex health care needs at various stages of the life cycle
Preferred Membership In Related Professional Organization

Up to 1 year experience clinical in a nursing unit‑specific area.

Job Responsibilities

Performs initial and ongoing assessment of patient and family, completes initial assessment tool, documents ongoing assessment per unit/Tri Health guidelines, and communicates assessment findings to other health‑care providers as appropriate. Includes health counseling and health teaching needs in assessment.

Plans care for patient and family based on assessment, standards of care, and optimal specific outcomes. Initiates and individualizes appropriate patient care guidelines/plan of care or clinical pathways, updates current plan of care as needed, and develops both short‑ and long‑term goals with patient/family and healthcare team including discharge planning.

Provides a safe, therapeutic environment, maintains patient’s autonomy, dignity, and rights, and is sensitive to patient diversity.

Seeks resources to help form ethical decisions, balances patient’s needs and those of the unit/facility, recognizes emergency situations and takes appropriate action, and completes patient assignment including documentation within scheduled timeframe.

Bases interventions on clinical data and desired outcomes and documents accordingly. Trains/educates other staff and acts as an expert resource in specialty area through continuous learning.

Effectively communicates information relative to surgical cases and carries out physician orders.

Evaluates the plan of care for patient based on optimal specific patient outcomes, documents patient/family response to care including teaching, and collaborates to revise plan of care as needed. Supervises care delegated to other health‑care team members.

Performs technical skills according to policy and procedure and accepted standards within their area of practice, safely administers medications/treatments and monitors their effects, and uses all equipment in a safe, appropriate manner.
