Job Summary
The Health Services Coordinator supports the health services team in delivering resident care and maintaining operations in an assisted‑living setting. Duties include assessment, individualized care planning, medication management, training, and coordination of services under the direction of the Director of Health Services.
Schedule & CompensationPart‑time, 10am – 6pm, Sunday – Tuesday Pay range: $35 – $40 per hour
Responsibilities- Conduct assessments and evaluations of potential residents; recommend admissions.
- Draft and update individualized service plans; review plans with responsible parties and the Executive Director.
- Maintain continuity of care for assisted‑living residents.
- Provide training and education on medication delivery, safety protocols, emergency procedures, and documentation.
- Serve as a medication technician when required; manage medication orders, central storage, and recording of administrations.
- Monitor vital signs and identify changes in resident health.
- Perform basic nursing functions, including treatments, medication delivery, and emergency response.
- Maintain inventory of care supplies and minimize waste.
- Coordinate prescription orders with physicians and pharmacies.
- Assist in marketing efforts and act as liaison between the community, families, and health providers.
- Ensure a safe environment for staff, residents, and guests.
- Promote teamwork and open communication in alignment with company values.
- Cover duties during absences and support the department as directed.
- Other job duties or special projects as requested by the Supervisor or Executive Director.
- RN, LVN, or LPN license active and in good standing; CEU and renewal requirements met.
- Minimum 2 years of related experience in a similar healthcare environment.
- Background clearance, health screening, and negative TB test within 7 days of employment.
- Valid state driver’s license and reliable transportation.
- Proficient in Microsoft Office Suite (Word, Excel, Outlook) and office equipment.
- Excellent communication skills in written and oral English.
- Sound decision‑making, calmness under pressure, professional demeanor.
- Strong organizational, problem‑solving, and time‑management skills.
- Ability to maintain a neat and organized work environment.
- Flexible schedule, including weekends and holidays.
- Mobility to walk, bend, kneel, squat and reach overhead; perform repetitive motions.
- Lift up to 50lb and push up to 250lb.
- Assist resident transfers and mobility during routine and emergency situations.
