Speech Language Pathologist – Elara Caring

Responsibilities- Promote the company’s mission, philosophy, and administrative policies to ensure quality of care.
- Evaluate patients to determine rehabilitation needs and develop a plan of care based on findings.
- Admit patients and assess speech‑language therapy needs according to OASIS and PPS regulatory guidelines.
- Conduct initial therapy evaluations, re‑evaluate patients during each visit, and perform follow‑up assessments.
- Initiate patient safety plans, coordinating with family and community resources.
- Communicate significant findings, problems, and changes in condition or environment to supervisors, physicians, and the care team.
- Participate in the implementation and development of the Plan of Care to ensure quality, continuity, and proper discharge planning.
- Verify the Plan of Care before each visit and provide care according to physician orders, assessment data, and established standards.
- Write physician orders for additional visits and changes to the Plan of Care.
- Incorporate patient care goals from the Plan of Care into therapy sessions, documenting in therapy notes.
- Perform direct speech‑language pathology services in accordance with accepted standards and certification by the patient’s physician.
- Select diagnostic and therapeutic materials appropriate to patient needs.
- Instruct patients and/or their caregivers in the home therapy program, providing modification and clarification as needed.
- Maintain accurate clinical notes and logs, complying with office policy.
- Operate in a physically demanding, high‑stress environment, handling lifting and transfers of patients.
- Potentially work irregular hours, including call hours if applicable.
- Master’s degree from an accredited educational program in Speech‑Language Pathology.
- Current, unrestricted SLP license in the state of work.
- Minimum of one (1) year of experience as a SLP in a clinical care setting.
- Willing and able to travel within branch/office coverage area, including 50% travel.
- Dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
- Ability to sit, stand, bend, lift, and move intermittently and lift 50‑100 lbs.
