Center Well Home Health is seeking a Home Health Physical Therapist to support patients at home, helping them regain strength, mobility and independence. You will report to the Clinical Manager and deliver personalized care focused on functional improvement within a collaborative care team.
Responsibilities include planning treatments, assessing strengths, documenting progress and guiding PTAs/HHAs, while communicating with physicians, patients and families. Minimum licensure and CPR required.
