Blue Springs Wellness & Rehabilitation is seeking a compassionate and driven Social Services Designee to support the emotional, social, and psychological well-being of our residents and their families. Key Responsibilities- Conduct psychosocial assessments and contribute to individualized care plans
- Support residents and families through transitions into long-term care and memory care
- Complete MDS-related documentation accurately and in compliance with regulations
- Advocate for resident rights, dignity, and self-determination
- Coordinate discharge planning and connect residents with community resources
- Collaborate with nursing, administration, and the interdisciplinary team
- Assist with survey readiness and regulatory compliance efforts
- Certified, licensed, or registered in the state of practice (required)
- Bachelor’s degree in Social Work or a related Human Services field
- Minimum of 1 year of social work experience in a healthcare or geriatric setting
- Skilled Nursing Facility (SNF) experience preferred
- Nursing experience in a SNF is a plus
