As the RN Care Plan Coordinator, you’ll play a key role in guiding resident care through the creation, coordination, and monitoring of personalized care plans. You’ll collaborate with a multidisciplinary team, including physicians, nurses, therapists, and social workers, to ensure that each resident’s medical, emotional, and social needs are fully addressed.

Care Plan Coordinator (RN)

Key Responsibilities:
  • Develop and implement individualized care plans tailored to each resident’s unique goals and health needs.
  • Collaborate closely with the care team to ensure care plans are carried out effectively and updated regularly.
  • Conduct comprehensive assessments and monitor progress, adjusting care plans as residents’ needs change.
  • Provide education and support to residents and families, helping them understand care plans and treatment goals.
  • Maintain accurate and timely documentation in compliance with state, federal, and facility standards.
  • Ensure care plan compliance with CMS, HIPAA, and North Carolina state regulations.
  • Participate in ongoing quality assurance and performance improvement initiatives.
Care Plan Coordinator

Qualifications:
  • Current Registered Nurse (RN) license in the state of North Carolina (required).
  • Strong organizational and communication skills.
  • Experience in long-term care or skilled nursing preferred.
  • Ability to work collaboratively in a team environment.
