The Licensed Practical Nurse (LPN) will oversee and support the healthcare needs of individuals receiving services. This position includes providing training, mentoring, and ongoing education to both new hires and current employees to ensure high-quality care and compliance with healthcare standards.

Responsibilities

• Monitor and support the medical needs of individuals served.
• Administer medications and treatments as prescribed.
• Coordinate care with physicians, pharmacies, and other healthcare providers.
• Maintain accurate medical records and documentation.
• Develop and implement individualized healthcare plans.
• Monitor for changes in health status and respond appropriately.
• Conduct medication administration and health-related trainings.
• Train and mentor new employees during orientation.
• Provide ongoing education and competency assessments for current staff.
• Ensure compliance with company policies, state regulations, and best practices.
• Participate in quality assurance and health-related program initiatives.
• Rotate on-call responsibilities (phone support only)
• 40 hours per week, Monday – Friday. No weekends, holidays, or night shift (other than on-call)

Qualifications

• CPR and First Aid certification, or willingness to obtain.
• Experience working with adults with Intellectual and Developmental Disabilities preferred.
• Training or supervisory experience is a plus.
• Strong communication, organizational, and leadership skills.
• Ability to work independently and collaboratively as part of a multidisciplinary team.
• Valid driver's license and reliable transportation.
