• Flexible scheduling:
  Per Diem | PRN | Part-Time | Contract

• 1:1 patient-focused care in a home setting

• Supportive clinical leadership

• Meaningful, relationship-driven work

• Opportunity to practice with autonomy and strong clinical judgment

• Conduct re‑assessments and supervisory visits

• Implement individualized plans of care

• Administer and document oral, topical, subcutaneous, and IM medications

• Monitor for and report adverse reactions to RN supervision

• Provide skilled wound care and initiate emergency measures when needed

• Maintain accurate, timely clinical documentation

• Perform all procedures within LPN scope and agency protocols

• Ensure professional, respectful communication with clients and families

• Uphold HIPAA standards and strict client confidentiality

• Graduate of a National League for Nursing–accredited program

• Active LPN license

• Minimum 1 year nursing experience (home care preferred)

• Experience with bowel & bladder care strongly preferred

• Current CPR certification

• Valid driver’s license & automobile insurance

• Negative TB test (or TB blood test)
