Location: 2 Westbury Park Way, Suite 102, Bluffton, SC 29910
Contract Duration: 13 Weeks
Guaranteed Hours: 32 hours/week
Shift: Monday to Friday, until shift completion
On-Call: Required as requested
Coverage Area: Hilton Head, SC
Productivity Requirement: 30 units/week
Job Summary:
We are seeking a dedicated Licensed Practical Nurse (LPN) to join our home health team in Bluffton, SC. This position involves providing skilled nursing care to patients in their homes, in alignment with their care plans. The ideal candidate will have experience in wound care, phlebotomy, and using various medical devices.
Key Responsibilities:
- Deliver compassionate, high-quality nursing care to patients in their homes.
- Document patient care and progress using Homecare Homebase (HCHB) and OASIS.
- Administer skilled nursing treatments such as wound care, wound VACs, trach care, Foley catheters, feeding tubes, ostomy, urostomy, and nephrostomy management.
- Coordinate care with interdisciplinary teams, patients, and caregivers.
- Participate in on-call rotation as needed.
- Active LPN license in the state of South Carolina.
- Minimum of 2 years' experience as an LPN in a clinical setting (home health or hospice preferred).
- Proficiency with Homecare Homebase (HCHB) and OASIS documentation.
- Current CPR certification.
- Valid driver's license and reliable transportation.
- Graduate of an accredited Licensed Practical Nursing Program or School of Vocational Nursing.
