What You’ll Do- Provide compassionate, hands-on care navigation and skilled nursing for clients in their homes
- Help older adults age gracefully and safely at home — avoiding unnecessary hospital visits
- Manage medications, coordinate care, and communicate closely with families
- Offer skilled nursing services such as IVs, wound care, blood draws, and catheter care
- Partner with leadership and medical teams to ensure clients receive exceptional care

Requirements
What You’ll Bring- Active RN license (Required)
- BLS and First Aid Certification
- A warm, professional presence and a passion for patient-centered care
- Excellent communication, organization, and problem-solving skills
- Experience with older adults or care coordination a plus
- Valid driver’s license & auto insurance
- Current CPR, annual TB test, and flu vaccine
- Clear background & drug screening
