Job SummaryProvide a patient onboarding experience for patients admitting to the in‑patient units and support units upon discharge. Assess, plan, implement, evaluate and document all aspects of patient care, working closely with all health care providers to facilitate efficient, effective, quality health care. Demonstrate knowledge of developmental principles across the life span, assess data reflective of patient status, interpret information to identify each patient’s requirements, and provide appropriate care to the relevant patient group(s).
Implement and comply with all policies, procedures and standards of the unit, department and institution.
- Coordinate care with physicians.
- Actively collaborate with other units and program staff, physicians, and members of other departments to facilitate provision of optimal patient care.
- Provide patient education while administering medication.
- Practice safe medication administration consistent with scanning of patient and medications.
- Scan all medications and patients using eMar.
- Follow facility established guidelines for reconciliation of medications on admission, during transfer and at discharge.
- Assign and conduct appropriate individual and group interventions within the milieu for behavioral and safe management of patients.
- Maintain appropriate boundaries and demonstrate least restrictive interventions appropriately.
- Collaborate with interdisciplinary team in assessing, planning, implementing and evaluating treatment, patient safety and programs.
- Assess patient’s response to treatment and safety interventions and accurately document interventions and patient’s response.
- Assist patients and families to understand the diagnosis, prescribed treatment and safe management of symptoms related to specific mental disorders through health teaching.
- Document against the interventions within the Recovery Plan.
- Utilize teaching skills related to psycho‑educational and medical interventions and patient safety needs with the patients and families.
- Assume responsibility for renewing license.
- Assume responsibility for completing all mandatory educational, health screening, CPR, and CPI requirements within designated time frame and for providing written verification for filing.
- Transcribe orders accurately and scan orders to pharmacy in a timely fashion.
- Provide the new patient with an in‑depth onboarding experience.
- Provide support to the unit staff with discharging patients.
- Registered Nurse (RN) with an Associate Degree.
- Basic Cardiac Life Support certification obtained within 30 days of employment start date.
- Non‑violent Crisis Intervention certification obtained within 30 days of employment start date.
- Three (3) years of experience in a mental health setting.
- No travel required.

Live Oak Mental Health and Wellness Center is a part of Trident Health. We provide inpatient stabilization treatment to patients with acute psychiatric symptoms that cannot be addressed on an outpatient basis.
