Responsibilities- Coordinate admissions of hospice patients by making skilled assessments of patients’ initial physical, psychosocial, and spiritual needs.
- Coordinate the establishment of the initial plan of care with the attending physician, medical director, and the interdisciplinary team.
- Provide skilled interventions and instructions to patients and caregivers to provide optimum symptom control and patient comfort under the direction of the physician.
- Function as a liaison between disciplines and facilities during the admission process.
- Develop, prepare, and maintain individualized patient care progress records accurately and in a timely manner. Submit accurate documentation per care center policy and record pain/symptom management changes/outcomes as appropriate.
- Communicate significant findings, problems, changes in condition or environment to the clinical manager/physician and other members of the hospice care team, and obtain physician orders as required.
- Perform on‑call responsibilities and on‑call services to patients/families as assigned.
- Perform other duties as assigned.
- Current RN license, specific to the state(s) you are assigned to work.
- One year of clinical RN experience.
- Current CPR certification.
