Job Description:

Administer skilled nursing care for clients of all ages in their place of residence. Assess home health and/or hospice patients to identify their physical, psychosocial, and environmental needs. Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care. Educate patients and/or their families regarding care, medications, and home safety. Supervise and provide clinical direction to the HHA/STNA/LVN staff.

Monitor assigned cases to ensure compliance with third party payer requirements. Provide effective communication as a liaison to patients/families and healthcare professionals.

Critical Information:
Shift: M-F until shift is complete; on-call responsibilities may be required. Remote/on-site:
On-site.

Education/Licenses Needed:
Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program. Current state license as a Registered Nurse. CHPN certification preferred. Proof of current CPR certification. 1-2 years experience as a Registered Nurse in a clinical care setting required. 2+ years of home health experience required.
