Registered Nurse – Home Health

Schedule:

8 hours

Requirements:

• Current state license as a Registered Nurse
• Healthcare software experience
• OASIS experience
• CHPN certification (preferred)
• Proof of current CPR certification
• Current driver's license
• Reliable transportation and insurance
• 1-2 years experience as a Registered Nurse in a clinical care setting
• 2+ years of home health experience

Roles & Responsibilities:

• Assess home health and hospice patients to identify their physical, psychosocial, and environmental needs.
• Implement and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
• Educate patients and their families regarding care, medications, and home safety, adjusting teaching methods as necessary.
• Supervise and provide clinical directions to home health aides and nursing assistants.
• Monitor assigned cases for compliance with third-party payer requirements.
• Perform on-call services as assigned.
