Registered Nurse – Home Health

Requirements:
- Current state license as a Registered Nurse
- Healthcare software experience
- OASIS experience
- CHPN certification (preferred)
- Proof of current CPR certification
- Current driver's license
- Reliable transportation and insurance
- 1-2 years experience as a Registered Nurse in a clinical care setting
- 2+ years of home health experience
Roles & Responsibilities:
- Assess home health and hospice patients to identify their physical, psychosocial, and environmental needs.
- Implement and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Educate patients and their families regarding care, medications, and home safety, adjusting teaching methods as necessary.
- Supervise and provide clinical directions to home health aides and nursing assistants.
- Monitor assigned cases for compliance with third-party payer requirements.
- Perform on-call services as assigned.
