Role Description This full-time, on-site Registered Nurse role is based in Bluffton, SC and focuses on providing skilled nursing care in the home setting. The Registered Nurse conducts comprehensive patient assessments, develops and updates individualized care plans, and delivers direct clinical care such as medication management, wound care, and disease management education. The role involves coordinating services with physicians, therapists, and other care team members to ensure safe, effective, and timely care.

The nurse provides patient and caregiver teaching on managing conditions, equipment use, and safety in the home, while documenting all care in accordance with regulatory and agency standards. Day-to-day responsibilities also include monitoring patient progress, responding to changes in condition, advocating for patient needs, and participating in quality and performance improvement activities.
Qualifications

• Active Registered Nurse (RN) license in the state of South Carolina and graduation from an accredited nursing program.
• Clinical skills in skilled nursing care, including assessment, care planning, wound care, medication administration, and chronic disease management.
• Strong patient and family education abilities, with clear communication, teaching, and counseling skills in the home setting.
• Experience collaborating with interdisciplinary teams, coordinating care with physicians, therapists, social workers, and other healthcare professionals.
• Proficiency in clinical documentation, use of electronic health records, and adherence to regulatory, safety, and infection control standards.
• Ability to work independently in the community, manage time effectively, and prioritize multiple patient visits and tasks.
• Valid driver's license, reliable transportation for home visits, and willingness to travel locally as required.
• Prior home health, acute care, or medical-surgical nursing experience preferred; certification in home health or related specialty is a plus.
