Registered Nurse (RN) Specialty Home Health
Schedule:
Shift: 8 hours
Requirements:
- Current state license as a Registered Nurse
- Homecare Homebase Software Experience
- OASIS Experience
- 1-2 years experience as a Registered Nurse in a clinical care setting
- 2+ years Home health experience
- Proof of current CPR certification
- Current driver's license and reliable transportation with insurance
Roles & Responsibilities:
- Assess and respond to the needs of patients and families in varied settings.
- Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Educate patients and/or families regarding care, diet, medications, home safety, etc.
- Supervise and provide clinical direction to healthcare assistants to ensure quality service.
- Arrange referrals to appropriate agencies when necessary.
- Monitor assigned cases to ensure regulatory compliance.
