Responsibilities:

• Administer skilled nursing care to patients in their home as prescribed by the physician and in accordance with the established plan of care.
• Assist in providing nursing services and identifying patients' physical, psycho-social, and environmental needs.
• Participate in planning, coordinating, and implementing care in conjunction with the RN to deliver quality, cost-effective patient care.
• Communicate effectively with patients, families, co-workers, and healthcare professionals, documenting outcomes accurately.
• Assist patients and families in learning appropriate self-care techniques and assess responses to care.
• Follow infection control measures to ensure safety for staff and patients.
• Support end-of-life care and assist with admitting, discharging, and transferring patients.
• Possible on-call duties.
