• Review and assess patient-specific therapies to ensure safety, appropriateness, and clinical effectiveness
• Develop individualized pharmacy care plans and support patients throughout their treatment journey
• Collaborate with physicians and interdisciplinary healthcare teams to optimize therapy outcomes
• Monitor laboratory values, evaluate patient progress, and make dosing recommendations when clinically appropriate
• Provide education and counseling to patients and caregivers regarding medications, infusion administration, and therapy expectations
• Oversee sterile compounding activities and ensure compliance with USP standards and regulatory requirements
• Support quality assurance initiatives and maintain accurate clinical documentation
• Serve as a trusted clinical resource for patients, caregivers, and healthcare partners
• Mentor pharmacy support staff and contribute to a collaborative, patient-centered culture

• Active Pharmacist license in the state of practice
• Bachelor of Science in Pharmacy, PharmD, or equivalent pharmacy degree
• Experience in acute care, home infusion, specialty pharmacy, or intravenous therapy settings
• Strong knowledge of infusion medications, sterile compounding, and clinical monitoring
• Ability to evaluate complex therapies and make evidence-based clinical recommendations
• Excellent communication and relationship-building skills
• Strong organizational abilities and attention to detail in a fast-paced environment
