The Patient Care Coordinator (PCC) plays a key role in supporting both the clinical and administrative functions of the outpatient clinic. This individual helps ensure smooth daily operations while delivering a positive and seamless experience for patients. Success in this role requires strong attention to detail, organization, and a patient-first mindset.
Primary Responsibilities Clinical Support- Measure and document patient vital signs, including blood pressure, pulse, respiration, and temperature.
- Conduct body composition assessments using InBody or similar equipment.
- Administer in-office point-of-care tests such as:
- Urinalysis
- Rapid strep, flu, and COVID testing
- Blood glucose monitoring
- Carry out phlebotomy procedures for blood sample collection.
- Coordinating and tracking referrals to specialists
- Communicating lab and imaging results to patients and providers
- Maintaining recall systems for preventive care and chronic condition follow-ups
- Collaborate with outside providers, specialists, and healthcare facilities to ensure continuity of care.
- Schedule appointments and maintain an organized clinic calendar.
- Prepare, update, and manage patient records and documentation.
- Respond to patient questions and assist with both clinical and administrative inquiries.
- Friendly, professional, and positive approach to patient care
- Strong verbal and written communication skills
- Self-motivated with the ability to problem-solve independently
- Collaborative team player
- Excellent organizational and time management abilities
- Working knowledge of medical terminology and standard clinical practices
- High school diploma or equivalent
- Completion of an accredited Medical Assistant training program
- Medical Assistant certification preferred
- Minimum of 3 years of experience in a medical assistant or similar role preferred
