The Patient Care Coordinator (PCC) plays a key role in supporting both the clinical and administrative functions of the outpatient clinic. This individual helps ensure smooth daily operations while delivering a positive and seamless experience for patients. Success in this role requires strong attention to detail, organization, and a patient-first mindset.

Primary Responsibilities Clinical Support
  • Measure and document patient vital signs, including blood pressure, pulse, respiration, and temperature.
  • Conduct body composition assessments using InBody or similar equipment.
  • Administer in-office point-of-care tests such as:
  • Urinalysis
  • Rapid strep, flu, and COVID testing
  • Blood glucose monitoring
  • Carry out phlebotomy procedures for blood sample collection.
Care Coordination & Patient Follow-Up
  • Coordinating and tracking referrals to specialists
  • Communicating lab and imaging results to patients and providers
  • Maintaining recall systems for preventive care and chronic condition follow-ups
  • Collaborate with outside providers, specialists, and healthcare facilities to ensure continuity of care.
Administrative Responsibilities
  • Schedule appointments and maintain an organized clinic calendar.
  • Prepare, update, and manage patient records and documentation.
  • Respond to patient questions and assist with both clinical and administrative inquiries.

Qualifications
  • High school diploma or equivalent
  • Completion of an accredited Medical Assistant training program
  • Medical Assistant certification preferred
  • Minimum of 3 years of experience in a medical assistant or similar role preferred
