Case Manager

You will assess needs, develop care plans, and guide patients through services across providers and settings. Partner with clinicians, monitor progress using digital tools, and document in EHR systems. Responsibilities

• Assess patient needs and eligibility for services using digital and in-person workflows
• Develop and update individualized care plans in collaboration with clinicians and patients
• Coordinate services across providers, facilities, and community resources
• Monitor patient progress and engagement through virtual care tools and EHR systems
• Educate patients and families on care plans, resources, and self-management
• Document all interactions and care activities accurately in EHRs
• Collaborate with an interdisciplinary team to resolve barriers to care
• Ensure compliance with HIPAA and organizational policies
• Use data and reports to identify gaps in care and improve outcomes
• Contribute feedback to enhance Quadrant's virtual care and patient engagement solutions
