Dedicated Home Health is growing in Palm Beach County, and we're looking for experienced Physical Therapists to join our team on a per-diem basis.

• Serve patients in Palm Beach County
• Work with an experienced home health clinical and operations team

• Current Florida Physical Therapist license
• Medicare-certified Home Health experience
• Experience completing home health evaluations, reassessments and discharges
• Strong clinical documentation and care coordination skills
• Reliable transportation and ability to travel within the assigned territory
• Commitment to timely, patient-centered care

As a Per Diem Home Health Physical Therapist, you will evaluate patients in their homes, develop individualized plans of care, provide skilled physical therapy interventions, assess progress and coordinate with physicians, nursing and other members of the interdisciplinary care team.
