Our team is growing, and we’re seeking motivated and skilled Physical Therapists to join us in delivering exceptional patient outcomes throughout Central Florida.

The Home Health Physical Therapist (PT) will provide skilled therapy services to patients in their homes, focusing on restoring function, improving mobility, and promoting independence following illness, surgery, or injury. The ideal candidate is reliable, compassionate, and experienced in home health care.

• Evaluate patients’ functional status and establish individualized care plans
• Provide therapeutic interventions in accordance with the physician’s orders and Medicare guidelines
• Educate patients and caregivers on safe mobility, exercises, and home programs
• Communicate effectively with physicians, nurses, and other members of the interdisciplinary care team
• Accurately document visits and patient progress in compliance with agency and Medicare standards
• Ensure continuity of care and timely completion of documentation

• Active Florida Physical Therapist (PT) license
• Current CPR certification
• Minimum of 1 year of experience as a PT (home health experience)
• Valid driver’s license, reliable transportation, and proof of insurance
• Strong organizational and communication skills
• Knowledge of Medicare documentation requirements
