We are currently seeking a dedicated and experienced Psychiatric Registered Nurse (RN) to join our home health team on a per diem basis. This field-based role focuses on delivering compassionate mental health nursing care directly in patients' homes. The ideal candidate will be clinically skilled, self-directed, and committed to supporting mental wellness, stability, and independence in the community setting.

• Conduct thorough in-home psychiatric assessments and ongoing evaluations.
• Develop personalized care plans in collaboration with physicians and interdisciplinary team members.
• Deliver skilled nursing care tailored to each patient's mental health needs, including education and medication management.
• Monitor patient progress, medication response, and behavioral changes; adjust care plans as needed.
• Document all visits and communications accurately and promptly in accordance with regulatory and agency guidelines.
• Coordinate care with internal clinicians, therapists, and external support services.
• Provide guidance and education to patients and caregivers on mental health conditions, medication adherence, and effective coping strategies.
• Respond appropriately to psychiatric crises or urgent needs within the RN scope of practice.
• Maintain up-to-date knowledge of Medicare guidelines, state regulations, and agency protocols.

• Current, active RN license in the state of Florida.
• 1+ years of recent psychiatric or mental health nursing experience (home health).
• Prior home health experience.
• Strong assessment, clinical judgment, and communication skills.
• Reliable transportation, valid driver's license, and auto insurance.
• Ability to work independently and manage a flexible per diem schedule.
