Lead RN-Console - Per Diem

Trustbridge Hospice, part of Empath Health, is seeking a caring and motivated RN to help ensure the delivery of high-quality, coordinated patient care through comprehensive management of Electronic Medical Record (EMR) workflows and effective clinical leadership. This role supports patients, providers, and interdisciplinary teams by promoting accuracy, collaboration, and continuity of care from admission through discharge.

Schedule:

Per Diem every Saturday & Sunday 5:00am – 1:30pm.

• Process and manage all EMR workflows from admission through discharge, ensuring accurate, timely, and complete documentation that supports continuity and quality of care.
• Serve as a professional clinical leader in assessing, planning, guiding, and evaluating patient care, applying sound nursing judgment and evidence-based practice to support positive patient outcomes.
• Interact effectively with management, physicians, and field staff using a teamwork-focused approach to coordinate care, resolve issues, and ensure alignment with organizational goals and patient care standards.

• Registered Nurse (RN) degree from an accredited college or university; ACEN or CCNE accreditation preferred.
• Current Florida RN license.
• Bachelor of Science in Nursing (BSN) preferred.
• Basic Life Support (BLS) Certification.
• Hospice Certification preferred.
• At least 1 year of recent experience in hospice, oncology, medical/surgical, home health, or public health nursing preferred.
• Expertise in support skills, palliative care principles, and patient/family education techniques.
• Professional demeanor: flexible, creative, assertive, articulate, nurturing, and self-nurturing.
• Experience with Homecare Homebase software considered a plus.
• Strong computer skills, including proficiency with clinical documentation systems.
• Ability to work effectively under stress and meet deadlines in a fast-paced environment.
