Holocaust Services Geriatric Case Manager

Ruth and Norman Rales Jewish Family Services is currently hiring a full‑time Holocaust Service Geriatric Case Manager. This position is responsible for assessing individuals and couples’ capabilities to remain safely at home or assist them in developing alternate living arrangements. The role assists in gaining access to needed medical, social, educational, psychological, and other essential services, assesses clients’ strengths and limitations, develops a plan of care, coordinates services, links to community resources, and monitors overall service delivery.

Effective communication with all service providers and family members is essential.

• Masters in Social Work
• LCSW preferred
• Minimum 2‑5 years experience in geriatric case management
• Effective verbal and written communication skills
• Demonstrates ability to resolve problems and provide assistance and teamwork where appropriate
• Strong organizational skills reflected in effective scheduling, planning, and meeting deadlines
• Requires flexibility with scheduling on an as needed basis
