The Social Services Coordinator plays a key role in supporting residents as they transition through the continuum of care. Working closely with the interdisciplinary team, this role focuses on assessing needs, coordinating resources and ensuring smooth transitions across Independent Living, Assisted Living and Memory Care.

• Assess residents' social, wellness and healthcare needs
• Support residents and families during transitions across levels of care
• Collaborate with interdisciplinary teams and attend care coordination meetings
• Follow up with residents after hospitalization or significant health events
• Maintain accurate and confidential documentation
• Promote internal services and wellness programs
• Develop and maintain relationships with internal and external resources

• Bachelor's degree or higher in Social Work or a related field required
• Experience in senior living, healthcare or resident services preferred
• Strong communication, problem-solving and interpersonal skills

This role requires the ability to walk, stand and bend throughout the workday. The employee must be able to communicate effectively and have sufficient hearing and vision, including close, distance, peripheral vision, depth perception and the ability to adjust focus. The position requires the ability to lift and/or move items weighing up to 25 pounds with appropriate assistance and equipment. Must be able to move throughout all areas of the community and may occasionally assist residents with transitioning from a seated to a standing position.
