Private Duty Caregiver

Lifecare Home Health -  - Responsibilities:
Supports a patients Plan of Care (POC) by monitoring and recording patient condition, providing support and personal services, and teaching families;
Monitors and assesses patient condition by observing physical and mental condition, intake and output, and exercise;
Performs patient assessments and collaborates with the multi-disciplinary care team to develop and implement a plan of care;
Provides housekeeping or laundry services, shopping or other household tasks, and prepares and serves meals and snacks;
Provides personal services as needed by the client such as bathing, dressing, and grooming
