Nurse Practitioner

About the Role

As a Nurse Practitioner (NP) or Physician Assistant (PA) at Conviva Senior Primary Care, you will be part of a collaborative, multidisciplinary team that includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care.

In this role, you will work onsite at our Med Center and Boerne clinics located in the San Antonio area and report to our Associate Medical Director.

• Provide comprehensive care to seniors following clinical standards.
• Collaborate daily with your care team to ensure coordinated, high-quality care.
• Manage referrals, hospital/SNF coordination, DME, and home health services.
• Document care accurately and efficiently with support from quality-based coders.
• Participate in on-call rotation and support growth plans at your center.
• Show passion for providing value-based care.

• Master's Degree in Nursing or completion of a PA program with board certification.
• Active, unrestricted NP or PA license in the state of practice.
• Must be screened for TB if selected for this role.

• 2 or more years of experience in value-based care, primary care or Medicare Advantage settings.
• Active DEA license and Medicare/Medicaid provider numbers, or ability to obtain them.

Scheduled Weekly Hours : 40 hours per week

Pay Range : $123,800 - $170,300 per year, plus potential bonus incentive plan based on performance.
