Primary Responsibilities

• Perform nursing assessments and develop, implement, and evaluate plans of care in conjunction with physicians, patients, and caregivers per agency policy and procedure.
• Evaluate the effectiveness of the plan of care, patient responses, and outcomes, revising plans as needed.
• Re‑evaluate patient nursing needs, including ongoing assessments using the OASIS data set at appropriate time points.
• Provide skilled nursing services requiring substantial and specialized nursing care, including medication administration, wound care, therapeutic treatments, and skilled teaching as ordered.
• Plan and implement patient health teaching and counseling regarding disease processes, self‑care techniques, and prevention, involving significant others to promote independence.
• Participate in health promotion and disease prevention activities.
• Initiate appropriate preventive and rehabilitative nursing procedures following the physician's plan of care.
• Coordinate services and scheduling with the Clinical Team Manager and Clinical Team Coordinator, including recommendations for additional home health services within 24 hours of Start of Care.
• Assess the patient's condition during each visit, communicate assessments daily to the Clinical Team Manager, and implement interventions to meet patient needs.
• Perform care‑management duties, including notifying physicians of changes in condition, obtaining orders, reassessing for recertification, supervising LVNs and home health aides, attending case conferences, coordinating care, and documenting discharge.
• Document all communications with patients, families, physicians, pharmacy, and other disciplines.
• Prepare a written plan for the Home Health Aide, if applicable.
• Express verbal and written communication in a clear, positive, and cooperative manner.
• Travel around San Antonio and surrounding counties as a field‑based position; required on calls, including weekdays and weekends; mileage reimbursement and shift differential are offered.

• Graduate of an accredited school of nursing.
• Current unrestricted Texas RN license or compact license.
• BLS certification.
• IV certification.
• At least 1 year of experience in home health care or a related environment.
• Experience with wound care and developed computer skills.
• Ability to move and transfer patients.
• Reliable transportation capable of traveling to client or patient sites within designated area.
• Completed tuberculosis screening and proof of immunity to measles, mumps, rubella, varicella, tetanus, diphtheria, and pertussis via lab confirmation, documented vaccination, or doctor’s diagnosis.

• Bilingual (Spanish).

Hourly pay ranges from $28.94 to $51.63 per hour for full‑time employment, based on local labor markets, education, experience, and certifications.
