About the Role
Clinical Care Team LPN at New York Cancer & Blood Specialists (NYCBS) in Bohemia, NY. Working 5/8.5 hour days, Mon‑Fri 8:30am –5pm.
Responsibilities- Monitor and manage patient communications, ensuring timely responses and appropriate action on messages related to patient care coordination.
- Conduct patient outreach and education, answering clinical questions within LPN scope of practice.
- Guide patients through their care journey by facilitating appointments, follow‑ups, and ensuring patients have access to necessary services.
- Assist centralized FMLA team with completion of forms requiring provider signature.
- Process and coordinate prescription refill requests in collaboration with healthcare providers, ensuring accuracy and timely delivery.
- Utilize Prescription Monitoring System (PMP) for refill requests of controlled substances, ensuring proper follow‑up is scheduled.
- Liaise with the revenue cycle team to provide patients with updates on authorizations and referrals.
- Arrange Peer‑to‑Peer conversations with providers to ensure authorization of testing and medications completed in a timely manner.
- Work closely with physicians, nurses, and other healthcare professionals to develop and implement individualized plans of care for the patient.
- Identify qualifying patients and enroll them in Chronic Care Management to support ER avoidance.
- Work closely with the triage team and APP to coordinate care for patients experiencing acute symptoms.
- Serve as a liaison between patients and the healthcare team, addressing concerns and ensuring that patient needs are met promptly.
- Ensure patients are educated on their healthcare needs to ensure they are part of their overall treatment plan.
- Maintain accurate and up‑to‑date patient records, documenting all interactions, care time, and outcomes in compliance with health regulations.
- Graduate from an accredited School of Nursing and hold an unencumbered New York license.
- Minimum one year of clinical nursing experience preferred.
- Excellent assessment, communication, and organizational skills.
- Proficient computer knowledge.
- Ability to plan, organize, develop, and interpret health data, goals, and objectives to provide quality coordination of care.
- Urology experience preferred.
- Starting salary: $32/hour.
