New York Cancer & Blood Specialists is seeking an LPN to join the Clinical Care Team in Bohemia, NY. You will support patient care coordination, education, and communications, working Mon-Fri 8:30am–5pm across a 5/8.5 hour schedule.
As an LPN, you will assist with referrals, prescriptions, and testing authorizations while collaborating with physicians, nurses, and care teams to develop individualized plans and enroll eligible patients in Chronic Care Management.
