Per-Diem Youth Skills Worker

The Youth Skills Worker engages children, who are at risk of or diagnosed with a mental health diagnosis, in individual and/or group skill building activities. The youth range from 5-21 years of age and live in Suffolk County. All services are provided in person in the youth’s home and/or community.

• Provide in person services to assist the youth in developing and applying skills in natural settings
• Provide hands-on and task oriented activities to align with the youth’s plan of care goals
• Promote advocacy skills to the youth being served
• On-going collaboration with the youth’s Case Manager
• Complete progress notes for all contacts.
• Operate from a strength based and family driven perspective
• Additional responsibilities as assigned to support program operations

• High School Diploma or equivalent
• Minimum of TWO years’ experience working with children in the mental, behavioral health or other human service field.
• Excellent verbal and written communication skills
• Proficient computer skills with MS Office
• Valid and clean New York State Driver's License
