The Opportunity

The Case Manager will deliver youth-focused gun violence prevention and community violence intervention programming for FCA's Empowerment Through Wellness to Prevent Gun Violence Initiative.

Responsibilities
  • Work as an onsite case manager and member of the team to support youth charged with gun-related offenses.
  • Provide individualized case management for all participants.
  • Make appropriate cross-system referrals.
  • Life

    Skills:

    Harm reduction, parenting, healthy habits, financial literacy, and planning.
  • Manage, build and create relationships with local agencies and service providers.
  • Assist youth in navigating systems including providing court advocacy and transportation to medical appointments.
  • Work with the court staff/resource coordinators, mentors and therapeutic counselor and assist them in providing case management services to their high‑risk program participants.
  • Coordinate case planning/ service delivery with assigned court staff, including Resource Coordinators, and make appropriate cross‑system referrals.
  • Engage with other community‑based service providers as appropriate to ensure a wraparound approach.
  • Be available to support the team in emergencies regarding incidents with staff or participants.
  • Adhere to all documentation and database requirements.
Qualifications
  • Bachelors Degree in related field with appropriate experience.
  • Bi‑lingual English‑Spanish required.
  • 2+ years of experience in working with at risk populations.
  • Lived experience required and connections with at‑risk Hempstead youth a plus.
  • Proficient computer skills with MS Office.
  • Valid and clean driver’s license.
