PRN Outpatient Program Therapist/Case Manager (LCPC, LPC, LCSW, LMSW, LMFT)

• Assist patients in meeting their biopsychosocial needs as part of social services functions.
• Conduct assessments, develop treatment plans, and facilitate group, family, and individual therapy.
• Provide discharge planning and support patients through therapeutic treatment skills.

PRN:
Monday through Thursday 4:00 p.m. – 8:00 p.m.;
Friday 9:00 a.m. – 3:30 p.m.

• Master’s Degree in Counseling, Social Work, or a clinically related mental health field.
• Trained in Crisis Prevention Instruction and CPR (paid training provided).
