Summary
The Diabetes Prevention Program (DPP) lifestyle coach will lead participants at various ministries through the year‑long lifestyle change program. They help participants develop and maintain the skills needed to adopt healthy eating and physical activity habits and provide support by giving information, encouraging progress, and working with the group to address any challenges or barriers that arise. The coach delivers and facilitates evidence‑based programs and interventions to prevent and improve management of chronic disease.

Requirements- High School Diploma or equivalent combination of education and experience to include a focus in public health, social work, business, advocacy or health administration.
- Previous experience with electronic medical record systems is preferred.
- Possesses strong analytical skills, leadership and team building abilities, excellent interpersonal and communication skills.
- Completely CHW training/certification and Basic Life Support (BLS) within 6 months of employment.
- At least one (1) year of community involvement experience, including volunteer experience.
- Ability to communicate in an open, non‑judgmental, empathetic, flexible and compassionate style. Strong and effective facilitation skills. Ability to listen to and engage others in one‑on‑one or large group discussions.
- Work requires ability to communicate orally and to hear or communicate with focus population. Bilingual skills to work with patients in their native language are preferred; certification as a medical interpreter/translator is a plus.
- Possesses a personal presence that is characterized by a sense of honesty, integrity, and caring with a willingness to support the mission, vision, goals, and values of Trinity Health.
- Facilitate sessions over the course of the 1‑year program (weekly sessions for 16‑26 weeks followed by monthly sessions for the remainder of the year).
- Reinforce and integrate session material and build on concepts by referring to previous sessions and discussions.
- Support participants by providing information, encouraging progress, and working with the group to address challenges or barriers that arise.
- Encourage goal setting on a weekly basis.
- Prepare for each session (i.e., review lesson plan and content for each session, provide feedback for participants on their food and activity trackers).
- Deliver the National Diabetes Prevention Program lifestyle change program as it was designed. Provide culturally relevant examples of food, types of physical activity, barriers, etc. while adhering to the key messages of the curriculum.
- Accessible to participants before and after sessions to answer questions and follow‑up on any questions that cannot be answered during class time.
- Follow‑ups with participants outside of class if they are unable to attend a class.
- Record weekly data for each participant for submission to the Diabetes Prevention Recognition Program (DPRP).
- Assist with identifying, engaging and enrolling individuals that may be eligible for CHW interventions and evidence‑based programs. Conduct and interpret assessments to learn patients’ social needs, reduce barriers and connect to appropriate resources and services.
