Metabolic Clinic Coordinator – Metabolic Navigation Program

We are launching a new Metabolic Navigation Program and are seeking a Metabolic Clinic Coordinator to support patient outreach, education, and navigation in a hybrid care model. This role is essential to helping patients access the care and resources they need to achieve their health goals.

• Monday-Friday, 8:00 a.m.
  
  -5:00 p.m.

Hybrid schedule:
Three days onsite, two days remote following orientation

• Hourly pay range: $28.00 - $33.00 per hour, based on experience and qualifications

• Support daily operations of the Metabolic Navigation Program
• Serve as an intake and coordination resource for patients entering the program
• Document intake notes and maintain accurate, timely records

• Identify and help reduce barriers to care for patients
• Provide education on program services, care pathways, and available resources
• Assist patients in navigating care across in-person and virtual settings

Use motivational interviewing techniques to support patient engagement

• Assist with program planning, workflow development, and ongoing improvement
• Collaborate with interdisciplinary clinical teams, community partners, and internal stakeholders
• Adapt to evolving responsibilities as the program grows

• Minimum 1 year of experience in healthcare coordination, clinic support, patient navigation, or care coordination
• Experience working directly with patients or clients

• Background in social services, community health, public health, or health-related community outreach
• Experience with motivational interviewing or behavior‑change support
• Care management or patient navigation experience
• Knowledge of insurance or experience navigating coverage and benefits
