e’s Health System, exceptional care starts with connection, coordination, and compassion. We are launching a new Metabolic Navigation Program and are seeking a Metabolic Clinic Coordinator to support patient outreach, education, and navigation in a hybrid care model. This role is essential to helping patients access the care and resources they need to achieve their health goals.
Schedule & Location- Monday-Friday, 8:00 a.m.
-5:00 p.m.
Hybrid schedule:
Three days onsite, two days remote following orientation
- Hourly pay range: $28.00 - $33.00 per hour, based on experience and qualifications
- Support daily operations of the Metabolic Navigation Program
- Serve as an intake and coordination resource for patients entering the program
- Document intake notes and maintain accurate, timely records
- Identify and help reduce barriers to care for patients
- Provide education on program services, care pathways, and available resources
- Assist patients in navigating care across in-person and virtual settings
Use motivational interviewing techniques to support patient engagement
Program Development & Collaboration- Assist with program planning, workflow development, and ongoing improvement
- Collaborate with interdisciplinary clinical teams, community partners, and internal stakeholders
- Adapt to evolving responsibilities as the program grows
- Minimum 1 year of experience in healthcare coordination, clinic support, patient navigation, or care coordination
- Experience working directly with patients or clients
- Background in social services, community health, public health, or health-related community outreach
- Experience with motivational interviewing or behavior‑change support
- Care management or patient navigation experience
- Knowledge of insurance or experience navigating coverage and benefits
