Schedule Information
- American Heart Association certification (BLS) required for all direct patient care personnel
- Must provide scanned copy of BLS certification with expiration date
- License/Certificate Verification must be dated within 30 days of start
- Fit Test required
- Trinity 10 Panel Drug Screen required within 30 days of start
- Trinity Code of Conduct Form required
- Trinity Confidentiality of Patient & Proprietary Information & System Security Agreement required
- Trinity Integrity and Audit Services Form required
- Trinity MVR (Motor Vehicle Report) required within 30 days of start
- Trinity Color Vision required
- COVID-19 Vaccine proof or declination required
- MMR vaccination proof required
- Hepatitis B vaccination proof required
- Influenza Vaccine proof or declination required
- 2 Step PPD OR IGRA (Quantiferon/Tspot) required
- Varicella vaccination proof required
- Physical/Health Statement from exam within last year required

- Trinity Criminal Background Check required within 30 days of start
- Trinity Education Verification required
- Trinity EPLS report required within 30 days
- Trinity GSA/OIG report required within 30 days
- Trinity Joint Commission Testing (CORE Exam within a year) required
- Trinity NSO report required within 30 days
- Trinity System Access Form must be completed one week before start
- Trinity Skills Checklist required for clinical contractors
- Tdap vaccine or declination required
