Met Council is seeking a Last Mile and Care Management Lead to coordinate care management and last mile logistics for the Social Care Network. The Lead will also support last mile coordination contractors, liaise with SCN management, and oversee managed care invoicing processes.
This is a temporary position anticipated to end on March 31, 2027; it requires sensitivity to NYC low-income communities, strong MS Office skills, and excellent communication and time-management abilities.
