, is hiring a mission-driven Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide high-quality geriatric psychiatry and behavioral health services in home, assisted living, memory care, and skilled nursing settings.

, this is an opportunity to build and manage your own panel while collaborating with primary care, palliative care, and neurology teams.
- Integrated,
continuum-based behavioral health model - Focus on dementia-related behaviors, depression, anxiety, psychosis, mood disorders, and complex psychiatric comorbidities
- Work within a collaborative interdisciplinary team
- Autonomy with strong operational and clinical support
- Clear pathway into clinical leadership and program growth for high-performing providers
- Comprehensive psychiatric evaluations & medication management
- Management of behavioral symptoms in dementia and cognitive decline
- Collaboration with PCPs, palliative care, and facility partners
- Caregiver education and relationship-based care
- Documentation, care coordination, and panel management
- Active PMHNP-BC license
- Experience in geriatric psychiatry, SNF, ALF, home-based care, or community psychiatry
- Strong diagnostic and medication management skills
- Ability to practice independently while contributing to a team-based model
- Leadership potential with interest in growing a successful patient panel
