Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) – Geriatric Behavioral Health | Mobile / Community-Based Care

Senior Doc, part of Brigade Health & Affiliates is hiring a mission-driven Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide high-quality geriatric psychiatry and behavioral health services in home, assisted living, memory care, and skilled nursing settings.

, this is an opportunity to build and manage your own panel while collaborating with primary care, palliative care, and neurology teams.

• Integrated, continuum-based behavioral health model
• Focus on dementia-related behaviors, depression, anxiety, psychosis, mood disorders, and complex psychiatric comorbidities
• Work within a collaborative interdisciplinary team
• Autonomy with strong operational and clinical support
• Clear pathway into clinical leadership and program growth for high-performing providers

• Comprehensive psychiatric evaluations & medication management
• Management of behavioral symptoms in dementia and cognitive decline
• Collaboration with PCPs, palliative care, and facility partners
• Caregiver education and relationship-based care
• Documentation, care coordination, and panel management

• Active PMHNP-BC license
• Experience in geriatric psychiatry, SNF, ALF, home-based care, or community psychiatry
• Strong diagnostic and medication management skills
• Ability to practice independently while contributing to a team-based model
• Leadership potential with interest in growing a successful patient panel
