Position Summary

Tru Health is seeking an advanced practice provider, either a nurse practitioner (NP) or physician assistant (PA) with experience in primary care or internal medicine, preferably with a geriatrics focus. The position offers the opportunity to improve outcomes for long-term care residents by collaborating with a team of providers and case managers. Providers primarily work within nursing homes or assisted living communities with residents enrolled in special Medicare Advantage plans, delivering personalized, coordinated health care that improves quality of life and reduces unnecessary hospital visits.

Responsibilities
  • Assess patients’ medical and mental health needs and provide a proactive plan of care that manages conditions and addresses barriers to care.
  • Ensure compliance with local, state, and federal regulations related to clinical services.
  • Prescribe medications and order lab work, diagnostic procedures, and consultations.
  • Monitor patients’ compliance and response to treatment and modify plans as needed.
  • Work closely with RN case managers as part of an integrated care team.
  • Exercise independent judgment to treat patients with acute and chronic conditions, consulting with a supervising physician as appropriate.
  • Collaborate with facility partners and health plan leadership to implement the national model of care in compliance with all federal and state regulations.
Work Experience, Credentials, and Education
  • FNP, AGNP, or PA license required.
  • Degree from an accredited APRN or PA program.
  • At least 3 years of experience in clinical nursing or rehabilitation with geriatric populations.
  • Electronic Health Records experience.
  • Working knowledge of Microsoft applications, including Word, Outlook, and Excel.
Supervisory Responsibilities
  • May be required to provide training and advice to facility staff.
