Contract Details- Duration: 13 Weeks (or 26 weeks option)
- Start: ASAP
- Schedule:
Monday–Friday - Shift: 8:00 AM – 5:00 PM (8-hour days)
- Hours:
40 hrs/week - Productivity requirement: 30 points/week
- $54 – $56/hr
- Weekly Pay: $2,160 – $2,240/week
- Active Idaho RN License required
- BLS Certification required
- Home Health RN experience required
- OASIS documentation experience required
- Homecare Homebase (HCHB) experience required
- Strong case management skills required
- Ability to meet productivity targets (30 points/week)
- Valid driver’s license and reliable transportation required (home visits)
- Provide skilled nursing care in patient homes
- Complete OASIS assessments and documentation
- Develop and implement patient care plans
- Coordinate care with physicians and care teams
- Educate patients and families on care plans
- Manage caseload and meet productivity requirements
- Provide hospice and home health support services
